Request Forms

• The Request Form is a multi-field form that allows visitors to enter their contact and general service or product information on a dedicated page that supports the site’s primary call to action (Request a FREE estimate, Request a Quote, Request Inspection, etc.)
• The type of Lead Generation strategy will align with the client’s stated expectation. "I want all the information I need to begin a conversation with prospect about generating a quote or estimate or setting up an appointment, walk through, inspection, consultation, etc."
  
  
  Note: this form is not restricted to the examples listed below. This strategy can be used for all applicable call-to-actions that ask the visitor/user/prospect to provide information so our client calls them back.

• Writer will add name of form (Request Appointment, Request Service, Request Inspection, etc) to header button, navigation label (page name), H3, and form button.  
• If applicable, writer will add the word "FREE" or "Free" to header button, H3, and form introduction copy (Request Free Quote, Request FREE Estimate, Request FREE Consultation).
• By default, blank production templates will include a page named Request <<Form Name>> with pre-loaded form section (located in Full Page section folder and named "Request Form").
• Writer will use this page for all Pro Sites clients in place of desktop, tablet and mobile Appointments pop-up and mobile link. 
• Writer will use this page for all Pro Sites clients if Request Appointment, Request Service, Request Inspection, Request Free Quote, Request FREE Estimate, Request FREE Consultation, and the like is defined as call-to-action.
• Writer will update navigation label (aka page name) accordingly.
• Writer will update title tag, meta description, and page URL (/request-appointment, /request-inspection, etc) per current process.
• NOTE:  Request <<Form Name>> page is not counted towards total page count.
• NOTE: Request <<Form Name>> page does not include a hero, nor should one be added.
• Writer (or Artist) will update Service or Product form label accordingly.  If applicable, writer may substitute Service or Product with something more specific to business and request like "Case Type" or "Project." 
• Header button, tablet and mobile buttons, call outs, and copy that asks user to Request <<Form Name>> will link to corresponding Existing page option rather than anchor tag.

• Writer (or Artist) will update form Service / Product dropdown options to mirror global nav labels. If navigation displays (from left to right) HVAC, Heating, Plumbing, Septic then the dropdown will display (from top) HVAC, Heating, Plumbing, Septic.
  
• By default, Service / Product dropdown will display "Other" as last option.
• Artist / Amends team permitted to update form layout, fonts, and color to match website color palette and design.  All layout types are permitted.
• Artist will update all website forms to match this updated design -- including Contact Us form

• Writer will update form introduction copy using the form Free Text field included with section. Writer update <<NOB>>, <<time frame>> (as indicated on questionnaire*), and <<CTA>>**.

• Writer adds page meta info
• Consider that there are two button / CTA / form options: Request _______ or Call Me Back (aka Request a Call Back)

• Copywriter completes the initial build of the form according to the questionnaire, including any additional fields as requested.
• Amends team permitted to add additional fields to form if requested (ex appointment date / time preference)
• DSC is responsible to inform client that the shorter the form the greater chance of submission. 
• Amends team permitted to include client provided privacy policy or disclaimer to the form if requested.   Similar to the form introduction copy, this text is inserted using the "Free Text" field type option.
• Ideally, Privacy / Disclaimer copy should be inserted below Message field and above Submission Button.
